If you are off work for less than 7 days you can self-certify using a form available from your employer. We also have a stock of these forms if required (just ask at reception). If you are off work for more than 7 days and require a fit note (previously known as a sick note) then please mention this to the receptionist when you contact us rather than just booking an appointment.
